Document Description
The California Department of Fish and Wildlife (CDFW) has executed Amendment No. 1 to Streambed Alteration Agreement number EPIMS-SLO-35869-R4 (Agreement), issued to Roxie Schultzcooks. The Agreement authorizes activities to correct a culverted driveway crossing. Activities include installing temporary erosion control measures; removing the driveway material, culvert rock headwalls, and existing culvert; installing utility sleeves under the crossing; realigning the replaced culvert and adding a second pipe to accommodate high flows; reconstructing the driveway and culvert headwalls; regrading and seeding slopes and disturbed areas; and removing erosion control measures. Amendment No. 1 changes the installation of utility sleeves under the culvert crossing.
