Document Description
The California Department of Fish and Wildlife has executed Lake and Streambed Alteration Agreement EPIMS Notification No. EPIMS-MEN-30312-R1C, pursuant to Section 1602 of the Fish and Game Code to Jennifer Formoso. This Agreement relies on the Notification materials and a CDFW site inspection on July 29, 2022. The project is limited to one encroachment to remediate an on-stream pond and restore approximately 50 to 60 feet of a stream channel. This will include reestablishing a two-foot-wide streambed at the native channel grade, pulling back perched fill, rock armoring the channel and the base of the banks through the restored stream reach with sufficiently sized rock to stabilize the channel and revegetating with native and/or non-invasive and locally sourced material.

There will be no significant adverse impact on endangered, rare, or threatened species, or their habitat pursuant to Section 15065.
