Extensive floodwaters caused an unnamed creek running through a 6 ft culvert under the roadway to top its bank and wash over road. Additionally debris clogged the culvert inlet creating water pressure which undermined the road and the slope embankment at the culvert inlet. Edge of roadway was undermined, scouring base, sub base and washing out embankment. The proposed project includes work performed to conduct road patching repairs, and clearing of fallen trees resultant from the storm.
