Document Description
The California Department of Fish and Wildlife has executed Lake and Streambed Alteration Agreement Number 1600-2015-0233-R1, pursuant to Section 1602 of the Fish and Game Code to Humboldt county Public Works Department (Permittee) as represented by Mr. Tom Mattson. The project is limited to one encroachment to restore 47 feet of roadway and roadside shoulder on Centerville road at mile Post 2.50 damaged during 2011 storm events, which involves reconstruction of the stream bank located immediately below the road. A vinyl sheet pile wall will be installed below bottom of the channel and recessed into the southern stream bank up to the ordinary high water (OHW) mark, and will be tied to roadway with series of 11-foot long anchors. The sheet pile wall will then be capped with a recessed gabion basket structure up to the 100 year-year flood elevation. The project will remove 15 cubic yards of road base and asphalt, 70 cubic yards of existing road fill, and 231 cubic yards of embankment soil to install the revetment structures. No stream flow diversion is required, fish will be isolated from site by a qualified biologist, and no trees will be removed and impacts are limited to herbaceous vegetation along road.

Reasons for Exemption
Storm repair activities will be conducted outside the wetted channel during low flow season. There would be no significant adverse impact on endangered, threatened, or rare species or their habitat pursuant to §15065. There are no hazardous materials at or around the project site that may be disturbed or removed. The project will not result in impacts that are significant when viewed in connection with the effects of past projects, the effects of other current projects, and the effects of probable future projects.
