Document Description
The project is located at 880 Oak Park, Assessor's Parcel Number 007-771-076, 053, 062, in the City Arroyo Grande, County of San Luis Obispo and would consist of building an outpatient surgery center. The project would be located on a 1.8 acre lot west of and adjacent to Meadow Creek, which tributaries to the Pismo Beach Ecological Reserve. A 25,000 square foot split level office building and associated parking spaces would be built at the site. Approximately 315 linear feet of retaining wall, just below the top of the Meadow Creek bank, would be built to support the parking area. No other construction would occur within the banks of Meadow Creek. The California Department of Fish and Game is executing a Lake and Streambed Alteration Agreement Number R3-200-0115 pursuant to Section 1603 of the Fish and Game Code to the project operator, Russ Sheppel.
