Document Description
The California Department of Fish and Game is executing a Lake and Streambed Alteration Agreement number 1600-2005-0164-3 pursuant to Section 1602 of the Fish and Game Code to the project applicant, Wesley Chow of Cuschieri Horton Architects, as agent for Kaiser Foundation Health Plan. In Phase I of this project the applicant proposes to construct a vehicular and pedestrian bridge across Calabazas Creek in Santa Clara County, California. Two riparian mitigation sites will be established and maintained on the Kaiser property. Phase II will be future construction of a second bridge across Calabazas Creek upstream from the first. The Department will issue a subsequent Agreement for Phase II of this project when design is complete in several years.
