Lake or Streambed Alteration Agreement No. 1600-2019-0410-R1,NOD,9/29/2022,9/29/2022,"The California Department of Fish and Wildlife has executed Lake and Streambed Alteration Agreement number 1600-2019-0410-R1, pursuant to Section 1602 of the Fish and Game Code to Victor Cornejo. The project is limited to two encroachments. The first encroachment is for channel improvements for fish passage on a segment of Denmark Creek which contains a defunct fish ladder and an approximately 210-foot long concrete lined channel. Final plans are to be determined, but will involve removal of the fish ladder, removal of concrete liner from the channel, restoration and stabilization of the stream channel, and facilitation of fish passage through the entire stream reach associated with the project. Plans may also involve modifications to the existing pond that is adjacent to the concrete lined channel in order to facilitate fish passage. Final basis of design and construction plans shall be submitted to CDFW for review and acceptance within three years of the effective date of this Agreement. The second encroachment is to upgrade an existing culvert. Work for this encroachment will include excavation, removal of the existing culvert, replacement with new properly sized culvert that is designed to pass salmonids at all life stages, backfilling and compaction of fill, and rock armoring as necessary to minimize erosion. If the to-be determined design of the channel improvement and fish passage project results in changes to the proposed design for the stream crossing, then updated final plans shall be submitted for review and acceptance.",https://ceqanet.opr.ca.gov/2016112028/335,Diaz Stream Crossings and Remediation Project Lake or Streambed Alteration Agreement No. EPIMS-MEN-29084-R1C,John Herrera,California Department of Fish & Wildlife,Environmental Scientist,john.herrera@wildlife.ca.gov,32330 North Harbor Drive,,Fort Bragg,CA,95437,7076533710,39°3'3.6"N 123°24'40.3"W,Booneville,Mendocino,Mendocino,approximately 3.8 miles northwest of the town of Boonville,,,046-130-17 and 046-130-05,,Navarro River watershed,,,,,,,,,,,,,California Department of Fish & Wildlife,No,9/9/2022,No,No,Yes,,Yes,No,No,No,"County of Mendocino 860 N.
